SIR,

IN PURSUANCE OF THE STATUTE 25 & 26 VICTORIA, CAP. 107, S. 12, I HAVE TO INFORM YOU THAT AT THE SESSION OF THE PEACE FOR THE COUNTY OF MIDDLESEX, HELD HERE ON THE 21st[3] INSTANT, A FINE OF £10[4] WAS IMPOSED UPON YOU FOR NOT ATTENDING AS JUROR IN OBEDIENCE TO A SUMMONS IN THAT BEHALF; AND I HAVE TO REQUIRE YOU WITHIN SIX DAYS AFTER THE DATE OF THIS LETTER, TO FORWARD TO ME, AT THIS PLACE, AN AFFIDAVIT IN WRITING OF THE CAUSE (IF ANY) OF YOUR NON-ATTENDANCE, IN ORDER THAT I MAY SUBMIT THE SAME TO THE COURT FOR ITS DECISION AS TO REMITTING THE FINE.

YOUR ARE PARTICULARLY DESIRED TO TAKE NOTICE THAT AS THE STATUTE ABOVE CITED REQUIRES AN AFFIDAVIT, NO OTHER COMMUNICATION FROM YOU BY LETTER OR IN PERSON CAN RECEIVE ATTENTION, NOR CAN ANY LETTER BE REPLIED TO UNDER ANY CIRCUMSTANCES; AND IF, AFTER THIS CAUTION, YOU DO NOT SEND ME AN AFFIDAVIT, THE FINE WILL BE LEVIED, ALTHOUGH YOU MAY HAVE WRITTEN ANY EXPLANATION BY LETTER TO ME.
